Reserve Form
Faculty are encouraged to work with the library to make sure that materials are available for their students.
You can even have the library make electronic reserves for materials that are available online or in the library's databases.
The library wants to support faculty and classroom instruction.
Reserve items can be personal copies owned by the individual teaching a class, or materials that the
library owns that require special access during the semester. Materials should be placed on reserve if a
limited 2 hour, 3 day, or 7 day checkout is needed to ensure that all students in a class have access to the
materials.
When placing photo copies of copyrighted material on reserve no more than one photo copy of any one
item may be placed on reserve unless copyright clearance has been granted by the copyright holder of the
material. Proof of copyright clearance must be provided to the library. If you need assistance in obtaining
copyright permission or if you have questions regarding copyright limitations, please contact Holly Hebert at

Please place materials on reserve no later than 24 hours before materials will have to be accessed. If the
library owns an item that is to be placed on reserve, additional time may be required to recall an item if it is
currently checked out. It is best to submit reserve requests at least one week prior to the semester that an
item will be used.
Please fill out the following form completely to make sure your reserve request can be processed. If you
do not provide all requested information, the library may not be able to complete your reserve request.
Submit only one item/request per form, i.e. one book, one article, one video, etc.
